2/17/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning observations, the client sat quietly in his seat and raised his hand before speaking while completing his morning stations. During midday observations, the client interacted with his peers in a positive manner while in the cafeteria for lunch. During afternoon observations, the client exhibited difficulty with remaining calm, complaint, and sitting quietly in his seat during instruction. QMHP-C greeted upon arrival to the school setting. During morning observations, QMHP-C provided the client with positive reinforcement and encouraged him to continue sitting quietly in his seat and to raise his hand before speaking. During midday observations, QMHP-C provided the client with verbal praise and encouraged him to continue interacting with his peers in a positive manner while in the cafeteria. During afternoon observations, QMHP-C redirected the client and reiterated the importance of remaining calm, compliant, and sitting quietly in his seat. The client responded well towards Therapeutic Day Treatment services as evidenced by positive participation and feedback. During morning observations, the client agreed to continue sitting quietly in his seat and to raise his hand before speaking while completing his stations. During midday observations the client continued interacting with his peers in a positive manner while in the cafeteria for lunch. During afternoon observations, the client was unreceptive to redirection by becoming extremely angry and locking himself in the restroom. The client was then able to calm down and agreed to remain calm, compliant, and to sit quietly in his seat during instruction. The client also agreed to continue to track is behavior on his self-monitoring chart.
